This consultation questionnaire does not guarantee your booking, it is designed to give me an idea of the project overall. Once I have received the questionnaire, I will contact you by phone and we will finalize an appointment and review any questions either of us may have. All appointments require a deposit, at the studio $60 is required, if done through website store $100 is required. We do accept all major credit cards.
